What is The “da Vinci” System

Hernia repair is the most common operation performed worldwide, with over 1 million repairs performed yearly in the United States alone. There are many types of hernias and they can occur at numerous anatomical sites including several different areas of the abdominal wall (umbilical, inguinal, and ventral just to name a few), to areas of the diaphragm, to hernias related to internal anatomy (e.g. internal hernia after gastric bypass surgery).

In addition to anatomic site variation, hernias range from simple first-time occurrences to those that have re-occurred once or several times and thus require a more extensive analysis of why this may have happened and subsequent planning for risk-factor reduction through an individualized plan for subsequent repair.

A hernia is the defect, or hole, through which another structure starts to push through. The hernia is NOT the bulge itself, but rather the gap through which something else is bulging outward or into a space where it does not belong. Hernia meshes are generally similar to mosquito net, but are sterile and more highly engineered to help provide mechanical reinforcement of tissue. Since a hernia is a defect, not an actual structure itself, repairing the hole often requires a patch, or piece of mesh, in order to close the gap and restore the natural integrity of the tissue layers. Sometimes small hernias can be repaired by simply sewing the edges of the hole together, but this only applies to very small hernia defects. For most hernias, a patch is required; experience has shown that repairing a large hernia defect without mesh leads to excessive tension on the tissues, which in turn leads to a guaranteed recurrence of the hernia over time.

Esophagus Function

The esophagus is the tube that carries food and liquids from the mouth into the stomach. The wall of the esophagus is composed of layers of muscle that propel food to the stomach by rhythmic waves of contractions called peristalsis. There are muscle rings at each end of the esophagus, – called sphincters. These sphincters open to allow food to move forward and then close to prevent food and acid from moving backward, helping prevent acid reflux and aspiration or choking.

Upon swallowing, the upper esophageal sphincter opens and the esophageal muscles contract to push food downward. The lower esophageal sphincter opens temporarily to allow food to pass into the stomach. Then the sphincter closes to prevent food and stomach acid from flowing back up the esophagus.

GERD, Barrett’s Esophagus, Esophageal Cancer, and Hiatal Hernia

Gastroesophageal reflux disease (GERD) is a common chronic disease where the acid from the stomach comes back up into the esophagus. The anatomical barriers that keep stomach acid from flowing upward are the lower esophageal sphincter and the diaphragm. Symptoms include heartburn ( acid indigestion), chest pain, regurgitation, and hoarseness.

A Hiatal Hernia is an opening of the diaphragm that allows part of the stomach to slide into the chest.  A hiatal hernia can contribute to or worsen symptoms of GERD. A hiatal hernia can be seen on an endoscopy, X-rays, and CT scans among other studies.

When GERD constantly irritates the esophagus, it can damage the lining of the esophagus and result in Barrett’s esophagus, a precancerous condition. About 8% of people with GERD go on to develop Barrett’s esophagus.

Esophageal cancer accounts for 3% of cancer deaths in the U.S. Symptoms of esophageal cancer include difficulty swallowing, painful swallowing, chest pain, worsening GERD, cough, or weight loss.  Risk factors include GERD, Barrett’s esophagus, obesity, smoking, and alcohol consumption. There are two primary types of esophageal cancer –adenocarcinoma or squamous cell carcinoma. Squamous cell carcinoma involves the upper and mid esophagus and is the most common esophageal cancer worldwide.  Adenocarcinoma involves the lower esophagus and is the most common type in the United States.

Achalasia is a rare disease of the esophagus where people have difficulty swallowing because the muscles of the esophagus fail to relax and allow food to pass into the stomach. It is believed to be caused by a malfunction of the nerves in the esophagus.

Both GERD and Achalasia are related to a malfunction of the esophageal sphincters.

Anatomy of swallowing food

When we swallow, our food travels down the esophagus to the stomach. When the food enters the stomach, a valve or ring of muscle called the lower esophageal sphincter (LES) closes to keep the food in the stomach.

What is Heartburn?

Heartburn is acid reflux that occasionally occurs when you eat spicy or acidic foods, fried or fatty foods, caffeine, and alcohol. Eating a large meal or eating before bedtime can cause these symptoms. Smoking and obesity contribute to the problem. More than 60 million Americans experience heartburn at least once a month. Heartburn can be treated with lifestyle changes and over-the-counter medications designed to decrease stomach acid production.

Typical heartburn symptoms include:

  • a burning sensation in the chest that can last for hours
  • a sensation of pressure or pain that gets worse when you lie down or bend over
  • an acid taste in the throat
  • a feeling that food or pills are stuck in your throat or the middle of your chest

What is Gastroesophageal Reflux (GERD)?
GERD is a chronic and often progressive disease where stomach acid and bile back up into the esophagus due to a weak LES. The acid irritates the esophagus and causes heartburn.

Persistent heartburn symptoms that occur several times a week and do not improve with over-the-counter medications and changes to your diet are GERD. In addition to heartburn, hallmarks of GERD are regurgitation and dysphagia, or a sensation that food is stuck in your esophagus.

GERD is a more serious condition that substantially impairs quality of life.

Left untreated, GERD can cause complications, including inflammation of the esophagus, pain, and a disease called Barrett’s Esophagus (BE), where the esophagus lining erodes. BE can lead to esophageal cancer. BE is more common in white men, people older than 50, smokers, and obese people.
