Introduction to Hernia Repair

Hernia repair is the most common operation performed worldwide, with over 1 million repairs performed yearly in the United States alone. A hernia is the defect, or hole, through which another structure starts to push through. The hernia is NOT the bulge itself, but rather the gap through which something else is bulging outward or into a space where it does not belong. Hernia meshes are generally similar to mosquito net, but are sterile and more highly engineered to help provide mechanical reinforcement of tissue. Since a hernia is a defect, not an actual structure itself, repairing the hole often requires a patch, or piece of mesh, in order to close the gap and restore the natural integrity of the tissue layers. Sometimes small hernias can be repaired by simply sewing the edges of the hole together, but this only applies to very small hernia defects. For most hernias, a patch is required; experience has shown that repairing a large hernia defect without mesh leads to excessive tension on the tissues, which in turn leads to a guaranteed recurrence of the hernia over time.

Esophagus function

The esophagus is the tube that carries food and liquids from the mouth into the stomach. It is made of layers of muscles that contract and relax sending food downward. There is a ring of muscles at each end of the esophagus, called sphincters. These sphincters open to allow food to move food into the esophagus, close to prevent food from coming back into the mouth and control the release of food into the stomach.

Upon swallowing, the upper esophageal sphincter opens and the esophageal muscles contract to push food downward. The lower sphincter opens temporarily to allow food to pass into the stomach. Then the sphincter closes to prevent food and stomach acid from flowing backward up the esophagus.

GERD, Barrett’s Esophagus, Esophageal Cancer and Hiatal Hernia

GERD is gastroesophageal reflux. It is a common chronic disease where the stomach contents back up into the esophagus. The anatomical barriers that keep stomach acid from flowing upward are the lower esophageal sphincter and the diaphragm. Symptoms are heartburn, chest pain, and hoarseness.

Squamous cell carcinoma is associated with alcohol and tobacco consumptions, some chemicals and vitamin deficiencies. Adenocarcinoma of the esophagus is primarily caused by Barrett’s esophagus.

Achalasia is a rare disease of the esophagus where people have difficulty swallowing because their throat muscles don’t relax and allow food to pay into the stomach. It is believed to be caused by a malfunction of the nerves in the esophagus.

In GERD and Achalasia, the problem is a malfunction of the esophageal sphincters.

Anatomy of swallowing food

When we swallow our food travels down the esophagus to the stomach. When the food enters the stomach there is a valve or ring of muscle called the lower esophageal sphincter (LES) that closes to keep the food in the stomach.

What is heartburn?

Heartburn is acid reflux that occurs occasionally when you eat spicy or acidic foods, fried or fatty foods, caffeine and alcohol. Eating a large meal or eating before bedtime can cause these symptoms. Smoking and obesity contribute to the problem. More than 60 million Americans experience heartburn at least once a month. Heartburn can be treated with lifestyle changes, and over-the-counter medications that are designed to decrease the production of stomach acid.

Typical heartburn symptoms include:

  • a burning sensation in the chest that can last for hours
  • a sensation of pressure or pain that gets worse when you lie down or bend over
  • an acid taste in the throat
  • a feeling that food or pills are stuck in your throat or the middle of your chest

What is gastroesophageal reflux (GERD)?

GERD is a chronic and often progressive disease where stomach acid and bile back up into the esophagus, due to a weak LES. The acid irritates the esophagus and causes heartburn.

Persistent heartburn symptoms that occur several times a week, and do not improve with over the counter medications and changes to your diet are GERD. In addition to heartburn, hallmarks of GERD are regurgitation and dysphagia or a sensation that food is stuck in your esophagus.

GERD is a more serious condition that substantially impairs quality of life.

Left untreated, GERD can cause complications including inflammation of the esophagus, pain and a disease called Barrett’s Esophagus (BE), where the lining of the esophagus erodes. BE can lead to esophageal cancer. BE is more common in white men, people older than 50, smokers and obese people.

Surgical Treatment Options for GERD

A laparoscopic Nissen Fundoplication (LNF) is surgery where the top of the stomach is wrapped around the LES to reinforce the sphincter and reduce acid reflux. LNF is the gold standard surgical treatment for GERD. However, it is a long and more invasive surgery, permanently alters the anatomy of the stomach, and is not reversible. Patients say in the hospital for 2-3 days, and recovery can take 4-6 weeks and involve dietary restrictions for a period after surgery.
